THIS week saw the launch of the new Hospice at Home scheme, organised by The Peace Hospice, which enables patients and families to receive hospice care while remaining at home.
The pilot scheme will operate in the Watford area and referrals can be made for any patients who attend surgeries attached to the Avenue and Garston clinics. The project involves collaborative work between the district nurses, Macmillan nurses and the hospice.
Sue Howse, lead nurse and hospice at home co-ordinator for The Peace Hospice said: "It's about keeping patients and families at the heart of all the decisions, considering what's important for them and providing care in the place of their choosing.
"It is hoped Hospice at Home will give more choice and support to those who wish to live independently and be cared for in their own homes.
"We will also be able to support the Primary Healthcare teams in maintaining patients and their families at home."
