A supported living service in Bushey was graded 'requires improvement' in a CQC report that found it breached residents’ human rights.
The Bushey Hall Road service, run by Achieve Together, provides care at a home for five people so they can live independently, although six were using it during the October inspection.
In a report published last month, the Care Quality Commission identified three breaches of care regulations in relation to consent, staffing and good governance.

The report said the service breached the residents’ human rights as staff used “restrictive practices” which had not been authorised by law, including locking the kitchen for “considerable lengths of time”.
The home also breached the 'good governance' regulations as it had been run by several managers who had not stayed in the post for long, leading to “a culture of poor leadership”.
It also failed to ensure staff received appropriate training as it was not clear if in-house personnel had properly completed the induction, and agency staff were told to read residents’ files which contained old information.
All assessed categories - safety, effectiveness, caring, responsive, well-led - were rated 'requires improvement'.
Achieve Together did not respond to a request for comment.
